Mycoplasma genitalium: The STI Behind Persistent Urethritis

Persistent or recurrent urethritis after common causes have been considered is a particularly useful clue, but laboratory confirmation is needed. This guide explains Mycoplasma genitalium symptoms, the names you may hear, and how testing can clarify overlapping signs. Mycoplasma genitalium, M. genitalium, Mgen or MG. It is not the same bacterium as M. hominis.

What causes Mycoplasma genitalium?

Mycoplasma genitalium is a small bacterium without a cell wall. It is also known as Mgen; MG. A laboratory result should be interpreted in context: detecting an organism does not always mean it is causing the current symptoms.

When symptoms may appear There is no dependable symptom-onset timetable for an individual. Infection can be asymptomatic, and symptoms may persist or recur.

Common symptom overlap chlamydia, gonorrhea, trichomoniasis, UTI, cervicitis and pelvic inflammatory disease

Symptoms: common does not mean specific

Possible findings include urethral discharge or irritation, burning urination, cervicitis, bleeding after sex, pelvic pain, or no symptoms. None of these, alone, confirms Mycoplasma genitalium. Symptoms may be absent, may appear later than expected, or may improve while the infection or underlying problem remains.

What can look similar?

The differential diagnosis includes chlamydia, gonorrhea, trichomoniasis, UTI, cervicitis and pelvic inflammatory disease. Coinfection is also possible, which is why a clinician may recommend more than one target or more than one sample site.

How is it diagnosed?

Use a validated molecular assay, such as PCR where appropriate, on an appropriate urine or swab sample. Culture is not practical because the organism grows extremely slowly. CDC does not recommend routine screening of asymptomatic people.

Testing detail: “STD panel” describes a group of targets, not one universal sample. Genital urine or swabs cannot reliably answer every throat, rectal, blood-borne or skin-lesion question.

When should you see a doctor promptly?

  • Severe pelvic, abdominal or testicular pain
  • Fever, faintness, rapidly worsening symptoms or pregnancy
  • A new genital ulcer, blister or widespread rash
  • Symptoms after sexual assault, or possible exposure to HIV
