If urethral symptoms keep returning, the next step is reassessment. Mgen is one important possibility, not an automatic diagnosis.

Everyday names and related terms
Mycoplasma genitalium, M. genitalium, Mgen or MG. It is not the same bacterium as M. hominis.
Persistent burning needs a fresh assessment
Persistent urethritis means symptoms and evidence of inflammation continue or return. Mgen is an established possible cause, but repeated symptoms can also reflect re-exposure, another infection or a non-infectious problem. How the result should be interpreted
Review the original tests, treatment and exposure sites. Mgen molecular testing may be appropriate for recurrent urethritis; resistance testing is useful where available. More antibiotics based on symptoms alone may not help. Clinical reference: CDC — Mycoplasma genitalium: STI Treatment Guidelines · CDC — Urethritis and Cervicitis: STI Treatment Guidelines
The important limitation
Persistent pelvic or perineal pain without objective urethritis may need a different evaluation. A broad panel is not a replacement for assessing pain syndromes or urinary disease.
The clinical question behind this search
Mgen is an established cause of persistent or recurrent nongonococcal urethritis. It should be considered after the sample site, timing, reinfection and other pathogens are reviewed.
| Cause | Mycoplasma genitalium |
|---|---|
| How it spreads / is acquired | sexual contact; evidence is strongest for genital transmission |
| When symptoms may appear | There is no dependable symptom-onset timetable for an individual. Infection can be asymptomatic, and symptoms may persist or recur. |
| Useful distinguishing clue | Persistent or recurrent urethritis after common causes have been considered is a particularly useful clue, but laboratory confirmation is needed. |
| Common symptom overlap | chlamydia, gonorrhea, trichomoniasis, UTI, cervicitis and pelvic inflammatory disease |
Symptoms: common does not mean specific
Possible findings include urethral discharge or irritation, burning urination, cervicitis, bleeding after sex, pelvic pain, or no symptoms. None of these, alone, confirms Mycoplasma genitalium. Symptoms may be absent, may appear later than expected, or may improve while the infection or underlying problem remains.
What can look similar?
The differential diagnosis includes chlamydia, gonorrhea, trichomoniasis, UTI, cervicitis and pelvic inflammatory disease. Coinfection is also possible, which is why a clinician may recommend more than one target or more than one sample site.
How is it diagnosed?
Use a validated molecular assay, such as PCR where appropriate, on an appropriate urine or swab sample. Culture is not practical because the organism grows extremely slowly. CDC does not recommend routine screening of asymptomatic people.
When should you see a doctor promptly?
- Severe pelvic, abdominal or testicular pain
- Fever, faintness, rapidly worsening symptoms or pregnancy
- A new genital ulcer, blister or widespread rash
- Symptoms after sexual assault, or possible exposure to HIV
Appearance and visual references
Appearance alone cannot confirm or exclude an infection. These photographs show a non-infectious change associated with propofol and urinary crystals. They are not photographs of vaginal discharge or a typical STI. Cloudiness by itself does not establish a UTI.
Questions readers ask
When is Mgen testing considered?
Particularly in recurrent urethritis or cervicitis; clinicians may consider it in PID.
Should persistent burning mean another antibiotic course?
Review the original tests, treatment and exposure sites. Mgen molecular testing may be appropriate for recurrent urethritis; resistance testing is useful where available. More antibiotics based on symptoms alone may not help.
A short summary for your testing decision
Review the original tests, treatment and exposure sites. Mgen molecular testing may be appropriate for recurrent urethritis; resistance testing is useful where available. More antibiotics based on symptoms alone may not help. For readers considering STD testing in Dubai, Qurapid’s Essential 14 panel is one option to discuss—not a diagnosis based on symptoms. Results are available the same day. Ask which sample and test timing fit your symptoms and exposure.
