Plain-language names used in this guide

Trichomoniasis is often called “trich”. Yeast infection is also called vaginal thrush or vulvovaginal candidiasis.

Cervicitis means inflammation of the cervix; urethritis means inflammation of the urine outlet. These describe a site of inflammation, not a single organism.

What can cause thick white discharge with itching?

Is it always an STD?

No. BV, candidiasis, irritation, hormonal discharge and urinary problems are not interchangeable with an STD. However, chlamydia, gonorrhea, trichomoniasis, herpes or syphilis may be relevant depending on the symptom and exposure. Because several are often silent, the absence of a “classic” sign does not rule them out.

When can symptoms appear?

There is no single STD incubation period. Trichomoniasis symptoms may appear in roughly 5–28 days or later; chlamydia symptoms may take up to about three weeks; herpes can be noticed within days or much later; and many infections never cause recognised symptoms. BV and yeast overgrowth do not follow a simple exposure clock.

Which test may help?

  1. Match the sample to the site. Vaginal or cervical sampling may be more informative for vaginal symptoms; throat and rectal exposure require site-specific swabs.
  2. Use molecular testing (such as PCR) where recommended. Molecular testing is preferred for chlamydia, gonorrhea, Mgen and trichomoniasis.
  3. Do not forget non-panel tests. Fresh sores are best sampled directly for herpes; syphilis usually needs blood tests; urinary symptoms may need urinalysis and culture; pregnancy-related symptoms need clinical assessment.
  4. Interpret positive colonisers cautiously. Gardnerella, M. hominis and Ureaplasma findings do not always prove they caused the symptom.
Before collecting a sample: tell the clinician about the date and type of exposure, antibiotics or antifungals, pregnancy, period timing, and whether symptoms are vaginal, urinary, oral, rectal or on the skin.

What not to do

  • Do not select antibiotics from discharge colour or odour.
  • Do not douche or use strong intimate washes to hide symptoms; they may worsen irritation or disrupt vaginal balance.
  • Do not assume improvement means an STI has cleared.
  • Do not rely on a urine sample for a throat, rectal or visible-lesion concern.
